POSITION PURPOSE The Care Coordinator provides comprehensive support for care management and care coordination activities for members enrolled in Medicaid and Medicare programs, including those receiving long-term services and support (LTSS) through waiver programs. This role manages a caseload of members, conducts in-home assessments, and collaborates with a multidisciplinary team to ensure integrated, high-quality care across the continuum. The position requires strong organizational skills, excellent communication, and the ability to work in a fast-paced environment while maintaining accuracy and compliance.

Monday through Friday - 8:00am - 5:00pm ; standard business hours with flexibility to meet service timelines.
